We will verify and update your information when scheduling an appointment. Please have your insurance information available.
New and existing patients should arrive 10 minutes early for your appointment to get checked-in. You will be updating information into your electronic health record via an iPad. Please bring a current medication list as well as insurance card, photo ID, and payment for services.

TEXT STATEMENTS: Beginning in March 2023 we will be sending out statements via TEXT message. You can follow the instructions for an easy and safe way to pay your bill. If you do not want a text statement; respond in the comment section of the text the word STOP. Your next statement will be a paper statement
Patient PORTAL: Appointment Reminders & Medical Record Access
Appointments: Every time you schedule an appointment an e-mail is sent to you with the details. Additionally, you are sent an e-mail reminder 2 days before your appointment.
If you need to set up your portal, call our office and request an e-mail link. The e-mail will come from Sadio.com.
New Medicaid Information:
We are no longer accepting patients with Medicaid insurance, unless you have Medicaid as a secondary insurance.

1. A “Total Body Check” /Annual Screening Exam

Unfortunately, the two appointments cannot be combined.
If you and your provider decide a procedure needs to be performed, an additional appointment may be required.
Procedures are generally applied to your deductible. If you haven’t met your deductible, you will be expected to pay towards the cost of the procedure at the time of your visit.
North Carolina law, G.S. 90-21.1, indicates a parent must give consent for any non-emergency treatment. Patients under the age of 18 must be accompanied by a parent.
In the event of inclement weather, a message regarding our office hours will be posted to our Facebook page as well as our phone greeting. You may call our office the day of your appointment to confirm our hours.
